Ensign Support
A referral form that never loses or doubles a referral
A support coordination business. Families, professionals and participants need to refer someone without a phone call, and the owner needs every referral in one place, handled carefully. We built the website and the software around it. We do not build or touch the regulated service itself.
Running on this page
Both sides of a referral, live
Not screenshots. Both are the client's own code, running here on invented people. They run themselves. Touch either one to take over.
1. The referral form families and professionals use
2. The private board the owner works from
What is running
What is behind the form and the board
Six mechanisms. One line each.
Five short screens instead of one long form
Who is referring, their details, the participant, the support needed, then a check. Each screen is checked before moving on, and someone referring themselves skips the questions that do not apply. So a referral takes minutes, not a phone call.
Saved before it says thank you, and never twice
Each referral carries its own id. The database refuses a second copy, a retry gets the same reference back, and the thank you screen only shows once the save is confirmed. So nothing is lost and nothing is doubled.
Only what is needed, nothing more
The form does not ask for plan numbers, diagnoses or files, and consent is enforced in the database. Alert emails carry a reference and a link, never the referral itself. So sensitive details stay in one private place.
A private board to work from
Referrals move from new to closed with a status history and private notes, behind a sign in that times out when idle and locks after repeated failures. So the owner can see at a glance what still needs a call.
Built to be used with a screen reader
Focus moves to each new screen's heading, and the first field with a problem is focused when something is missing. So people using assistive technology can refer someone as easily as anyone else.
A check that catches gaps before launch
A launch check, run before release, fails while any placeholder, sample data or draft label is left in the site. So the gaps you can see in the copy above are caught before the site is public.
What this makes possible
Two sides of the same build
For the person referring
- Refer someone at any hour, in a few minutes, without waiting on a phone line.
- Only be asked what applies to them.
- Know the referral arrived, with a reference to quote.
For the business
- Every referral in one private place, none lost and none doubled.
- A status and a history for each one, so nothing is forgotten.
- A morning nudge for referrals still new, once email is connected.
Not finished
In build, not yet public
The site is being finished and is not public yet. Contact details, the privacy wording and some headlines are still placeholders, which is why they show as gaps in the copies above.
Worth saying plainly
No one client has done the whole journey yet
Every piece of it is live somewhere, and we can show you each one.
