Restore and Beyond
Referrals, feedback and complaints, none of them lost
A psychosocial recovery practice. Support coordinators and families need to send a referral without a phone call, anyone needs to be able to complain without giving a name, and the practice needs every one of them in a private place. We built the website, the forms and the staff inbox. We do not build or touch the regulated service itself.
Running on this page
Both sides of a referral, live
Not screenshots. Both are the client's own code, running here on invented people. They run themselves. Touch either one to take over.

What is running
What is behind the forms and the inbox
Six mechanisms. One line each.
Three forms, one page, only the questions that apply
Referral, feedback and complaint sit behind three tabs, and a link can open any one of them. A private booking never sees the NDIS plan questions, and they only become required when the person chooses NDIS funded. So nobody is stopped by a question that is not theirs.
A referral in three short steps
What is needed, the participant, then who is referring. Each step is checked before moving on, and the rest is gathered at the first meeting. So a referral takes minutes, not a phone call.
Never silently lost
A form is saved to the database before the page says thank you. If the save cannot be reached, the visitor's email app opens with everything they typed, and the page gives the practice's number. So a referral is never dropped without anyone knowing.
Feedback and complaints without a name
Only the message is required, so either can be sent anonymously. The complaint form says plainly that people can go straight to the NDIS Commission instead. So raising a problem costs nothing.
A private inbox with a record of who read what
Each staff member signs in with their own account, which locks after repeated failures and times out when idle. The list shows summaries only, and opening a submission is logged against the person who opened it. So health information is seen by the right people, and it shows when it was.
Alerts that carry no health information
The practice gets an email when a form arrives, and the sender gets a thank you. The alert carries a name and contact details only, and sends after the save, so a mail outage never loses a referral.
What this makes possible
Two sides of the same build
For the person reaching out
- Refer someone at any hour, in a few minutes, without waiting on a phone line.
- Only be asked what applies to them.
- Give feedback or complain without giving a name.
For the practice
- Every referral, feedback note and complaint in one private place.
- A clear picture of what is still unread.
- A record of who opened each submission, for when it is asked for.
See it yourself
The site is public
The three forms are on the contact page. The staff inbox is private, so the copy above is the only way to see it.
restoreandbeyond.com.auWorth saying plainly
No one client has done the whole journey yet
Every piece of it is live somewhere, and we can show you each one.
